- Two passerby discover a severed head in a pond near a park. Dylan and Lizzie investigate and find there are three additional severed heads in the pond, but no sign of the bodies. The coroner finds that the heads were all put in the pond at different times, and further finds that all four of the heads have s specific chemical in them. Julian is asked to help with some computer investigation once the identity of the most recent head is known, This leads to a strange medical experiment and a possible mad scientist. Meanwhile, Dylan has a visit from someone from his past.
- Case of the week:
Two men walk in a park and see what seems to be a swimmer in a pond. One of the two dives in and touches the person in the pond, discovering there's only a head. Reinhardt and Needham are called in while divers are searching for the body; the divers find four heads in total. At the morgue, the ME informs them the heads were deposited in the pond between eight months and a few days and that the most recent victim is a gang member with a long rap sheet.
The gang leader tells Reinhardt and Needham that this gang member had plans to leave the gang and that he was "involved in something big in a medical place." The medical place turns out to be a study center for advanced cognitive functions. A doctor Portman and his wife (and fellow researcher) tell that the gang member was a custodian, but was fired. Facial recognition pictures are created for the other three heads.
In the research center, Reinhardt interviews the wife and discovers she has early onset Alzheimer's disease; her case was the cause of all research being changed to Alzheimer's research. Also, there had been a scandal when the doctor had experimented on decapitating pigs.
Back at the precinct, it is known that all heads were severed using a sharp object by a person with expertise on the matter. The police obtains a search warrant and finds studies on "death waves" using human subjects on the computer of Mrs. Portman. Both Dr. and Mrs. Portman deny any involvement.
Using "Jules'" advanced facial recognition software, all three other faces are linked to patients of the hospital who were suffering from a terminal condition. Reinhardt and Needham discover that at least one of the other patients was involved with Dr. Wells and search his office, where a working guillotine is found.
Dr. Wells enters and effectively confesses his crime, saying that he was working on breakthrough research and that all patient had consented to working on his study. Nevertheless, he is arrested for murder.
Grand arc:
Having seen a picture of an old enemy "Pasternak"in the previous episode, Reinhardt is having visions of that enemy while in his classroom and also suffers from blurry eyesight. He schedules an MRI and this MRI is interrupted by this Pasternak who reveals he had nothing to do with Maja's death or the infiltration in the precinct. He is willing to help stop the enemy, provided that Julian is willing to work with him. Julian refuses at first, but in the end is seen entering an unknown building with Pasternak to perform some activity (this arc is left unresolved by the ending of the series).
The eyesight issue is revealed to be stress-related and is gone when Reinhardt makes peace with Pasternak.
Lizzie Needham decides to take two weeks off and do a road trip, traveling across the country to Las Vegas where she is planning to watch a MMA fight with Detective Ryan Stock. The final scene of the series shows Reinhardt taking care of Lizzie's dog.
The "baby arc" is not touched on in this episode and is left unresolved.
Detective Ryan Stock says farewell to the precinct crew, but then decides to crash in the locker room and do some sightseeing of New York.
