This episode was written by Heide Perlman, Rhea Perlman's real life sister.
At the beginning of this episode, Carla (Rhea Perlman) is being carried out of the bar by Sam because she's going into labor. As she's going out the door, she announces she'll be having her baby at St. Eligius Hospital. This is the name of the fictional Boston hospital featured on another NBC series, St. Elsewhere (1982). In Cheers (1985), St. Eligius doctors Donald Westphall (Ed Flanders), Mark Craig (William Daniels) and Daniel Auschlander (Norman Lloyd) visit Cheers and meet Carla, Cliff and Norm. Carla recalls having her baby at the hospital, where she had a negative experience, and refers to the hospital by its derogatory nickname "St. Elsewhere." It is also revealed that Norm is Dr. Auschlander's accountant.
Paul Willson's second appearance, this time not as Glen or Gregg (per the credits) but tandom Bar patron, Tom. Years later he morphed into Paul, while Paul Vaughn, the original Paul, disappeared.
The principal guest character in this episode is Carla's younger sister (Rhea Perlman in a dual role). In the Season 1 episode, "The Tortelli Tort," Carla said that her sisters were all older than she.
